This will allow us to take steps to preserve fertility, if appropriate.<\/li>\n<li>In the case of a past diagnosis, after the cancer treatment has finished, you will be asked to send all documents related to your cancer diagnosis and treatment, as well as a letter from your oncologist describing the latest monitoring tests and\/or current medication.<\/li>\n<\/ul>\n[\/mk_custom_list][mk_fancy_title color=&#8221;#4a76c9&#8243;
